Universal Health Access, Inc is committed to protecting the privacy of our clients' and their personal health information. Part of that commitment is complying with the Privacy Rule of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), which requires us to take additional measures to protect personal information and also requires that all the medical records used or disclosed by Universal Health access (or any other healthcare provider) electronically, on paper, or orally, be kept confidential, and to inform our clients about those measures. Medical information is extremely personal and Universal Health Access, Inc will not use or release personal health information except as otherwise permitted and/or required by HIPAA.
The healthcare information is your personal
Your healthcare information is private and personal. We only obtain your health care information in order to process the ordering of your Lab tests, we must create certain records that contain information about you and the for your tests results. These records include your name, test/s ordered, results and billing records. The law requires that we give you notice of our privacy practices, and requires that we follow the terms of our privacy notice currently in effect. This Notice of Privacy Practices describes our commitment and the commitment of the physicians to the protection and confidentiality of your health information. This notice also describes your rights concerning your health information, including your right to request that we correct your current information, you may ask us to amend the information if it is incorrect.
Disclose your Health Information:
We are committed not to disclose your protected health information unless we have your written authorization and request. You may revoke the authorization permission in writing, and we are required to honor your request, except to the extent we have already disclosed information pursuant to the authorization.
We used limited Information to obtain payment for services
We used limited information in order to obtain payment through Credit card/ or money orders or through a check processing services. We do not share any information with your Credit Card Company or check processing service that discloses the type or the reason of the payment made by our customers.
Disclose the medical records
We may use and disclose your medical records for treatment, and healthcare operations, Public Health activities and disclose as required by law. (We may be required to disclose protected health information for public health activities to a public health authority authorized by law to collect or receive this information, such as the FDA (Food and Drug Administration), for the purpose of preventing or controlling disease, injury, or disability. We may use or disclose protected health information if required to do so by federal, state, or local law. The use or disclosure will be made in compliance with the law, and will be limited to the relevant requirements of the law.
The Test and Results You are the only one who get the results to unless you specifically direct us otherwise. Please note that for HIV testing, some state laws require that a positive result be reported. However, Universal Health Access will simply report that result as "positive" whenever possible and will take extraordinary steps to keep your name and other identifying information (such as date of birth, gender, etc.) out of that report. This is our mission, our goal, and our promise to you to protect your privacy. Please note that some states require reporting of other positive lab results, particularly those related to sexually transmitted diseases. Universal Health Access is bound by law to comply with state and federal requirements, and must do so when required by law.
The following rights pertaining to your personal health information are included in HIPAA:
You have the right to ask for restrictions on the ways your health information is used or to whom your information is disclosed, even if the restriction affects your treatment, our payment, or health care operation activities.
You have right to request that we communicate with you in a certain way or at a certain location. For example, you can ask that we only contact you by U.S. mail at a private post office box. We will not ask you the reason for your request.
You have the right to inspect and obtain a copy of health information that we maintain about you. This includes copy of the test records and billing records. To inspect or request a copy of your health information please contact our Privacy Officer as described later in this Notice. In certain very limited circumstances, the law provides that we may deny your request to inspect or copy these records. If you are denied access to health information, you may request that the denial be reviewed by a licensed health care professional chosen by us who did not participate in the original decision to deny your access to review your request and the reasons for the denial.
You have the right to request that we correct your current information, you may ask us to amend the information if it is incorrect. We are not required, however, to honor your request if we did not create the information you are requesting be amended or if the information in your record is correct. We will respond to your request in writing within 30 days of the date of receipt of your written request for amendment of your information, unless we advise you we require an additional 30 days.

When you place orders on UHA site, all of your order information, your card number and delivery address, is transmitted through the Internet using Secure Sockets Layer (SSL) technology. SSL technology causes your browser to encrypt your order information before transmitting it to our secure server. SSL technology, an industry standard, is designed to prevent someone other than operators of our site from capturing and viewing your personal information.
