You should understand that by ordering any of our Lab test Services you agree to be bound by these Terms and Conditions and all transactions with us.
Universal Health Access, Inc is an online lab test service. By purchasing online lab test service you agree to be bound by the following terms & conditions.
- You understand and agree that during the course of purchasing online test service, the information submit is accurate and complete.
- You understand and agree that use the online lab services for informational purpose only
- You agree that not to disclose your password to any other users
- You agree to abide by the order policy and refund policy
- You understand and agree that a lab test purchase online can not substitute medical advice, diagnosis and treatment.
- You agree that UHA shall not responsible for nay injury or illness that results from Blood drawn in any PSC.
- You also agree that lab test results can be false negative and false positive, which require repeat testing at additional cost.
- You agree that you are over 18 year old.
- Universal Health Access shall not be liable for any cost or damage either directly or indirectly from any transactions conducted or information, interpreting or reporting any laboratory data conveyed on the website
- Universal Health Access offers no assurance of uninterrupted or error free service and does not warrant and assurance of the complete accuracy of the information and material in the form of text, graphics, links or other items contained on the website.
- You agree to indemnify, release reliability and hold harmless (UHA) Universal Health Access Inc. and its officers, directors, employees, their affiliates, representatives, agents, and any third party information providers to the Lab test Service from and against all causes of actions, suites, penalties, liens, liabilities, obligations, losses, which may arise at any time by reason of, relating to, arising directly or indirectly related to lab tests results and services.
